Overview

This is an exempt role dedicated to the practice of critical care medicine, overseeing all facets of inpatient medical treatment under the appropriate physician supervision as mandated by New York State regulations. Within the collaborative relationship between the physician and the advanced practice provider, the Physician Assistant/Nurse Practitioner will exercise independent judgment in medical decision-making, alongside a comprehensive array of diagnostic and therapeutic interventions.

Key Responsibilities Include:

  • Adhering to the performance standards, policies, and conduct expected by White Plains Hospital.
  • Gathering medical histories, performing physical examinations, ordering and interpreting diagnostic tests, prescribing medications, and managing illnesses while consulting with physicians as necessary.
  • Offering consultation services for potential critical care patients from the emergency room, hospital floors, or post-operative care.
  • Actively engaging in daily rounds to establish treatment plans and objectives for each patient within the critical care unit.
  • Communicating treatment plans and other relevant matters with the families of critically ill patients.
  • Acting as the first responder to any patient in the critical care unit exhibiting significant changes in clinical condition.
  • Coordinating necessary treatments, interventions, diagnostic tests, or procedures for patients.
  • Facilitating appropriate consultations on behalf of the intensivist team.
  • Providing timely updates to the on-call intensivist regarding patient progress, acute changes, or challenges faced in clinical management.
  • Performing procedures within the scope of practice for the advanced practice provider.
  • Advising patients and families on healthcare matters, including discussions surrounding end-of-life care.
  • Documenting patient information and care in the medical records, which may involve maintaining departmental databases for research and quality improvement purposes.
  • Demonstrating knowledge in anatomy, pharmacology, pathophysiology, clinical medicine, and physical diagnosis.
  • Exhibiting proficiency in collecting and analyzing physiological, psychological, and socioeconomic patient data.
  • Effectively evaluating patient issues in person or via phone, providing suitable advice, interventions, or referrals.
  • Making informed decisions within the boundaries of PA practice.
  • Facilitating the care and management of patients admitted to the critical care units.
  • Performing additional related duties as assigned.

Educational & Experience Qualifications

For Nurse Practitioners:

  • Master’s Degree in Nursing from an accredited institution (strongly preferred).
  • Current New York State license as a Registered Professional Nurse.
  • Current New York State certification as a Nurse Practitioner (Adult/Geriatric).
  • Maintenance of a current collaborative practice agreement.
  • Current National Certification.
  • Advanced Cardiac Life Support (ACLS) certification.

For Physician Assistants:

  • Active New York State Licensed Physician Assistant.
  • Current National Certification.
  • Current White Plains Hospital Physician Assistant Delegation of Authority.
  • Master’s degree from an accredited institution (strongly preferred).
  • Advanced Cardiac Life Support (ACLS) certification.

Proficiency in all procedures, techniques, and skills as outlined in the delineation of privileges.

Core Competencies

Required Procedural Skills:

  • Insertion of central lines (triple-lumen or introducer) and dialysis catheters.
  • Insertion of arterial lines (radial, femoral, axillary).
  • Placement of nasogastric (NGT) and orogastric (OGT) tubes.
  • Insertion of Foley catheters.
  • Removal of surgical drains, dressings, and staples.
  • Participation in training programs if lacking procedural skills.

Knowledge Base Requirements:

  • Management of various types of shock.
  • Goal-directed fluid resuscitation techniques.
  • Administration of vasopressor and inotropic agents.
  • Hemodynamic monitoring practices.
  • Ventilator management and weaning strategies.
  • Understanding of sedation and paralytic agents, including monitoring techniques.
  • Daily sedation interruption and weaning evaluations.
  • Continuous venovenous hemodialysis (CVVHD) management.
  • Management of electrolyte and acid/base imbalances.
  • Arterial blood gas (ABG) interpretation.
  • Intracranial pressure (ICP) monitoring.
