Community Outreach Specialist

As a HOPE Team Outreach Specialist at Aspire Health Partners, you will play a vital role in providing essential services to individuals experiencing homelessness. Your primary responsibility will be to conduct outreach services, assessments, and referrals to support clients in reintegrating into their communities.
Key Responsibilities- Outreach Services: Provide direct support to clients in need, including those who are homeless or at risk of homelessness.
- Assessments and Referrals: Conduct thorough assessments to identify clients' needs and provide referrals to relevant resources and services.
- Document Assistance: Assist clients in obtaining necessary documents to access housing and other essential services.
- Criminal Justice System Navigation: Advocate for clients within the criminal justice system, seeking diversion when appropriate.
- Contractual Obligations: Ensure adherence to contractual requirements and agency policies.
- Community Representation: Serve as a representative for Aspire Health Partners in community planning and committees.
- Training and Meetings: Participate in required trainings and meetings to maintain professional development and stay up-to-date on industry best practices.
- Background Clearance: Maintain a Level II background clearance.
- Driver's License: Possess an acceptable Motor Vehicle Registration driver's license record.
- Education: High School Diploma required; Bachelor's Degree preferred.
- Experience: 1-2 years of experience working with targeted populations may substitute for higher education.
