Job Summary

As a HOPE Team Outreach Specialist at Aspire Health Partners, you will play a vital role in providing essential services to individuals who are homeless, facilitating their reintegration into the community.

Key Responsibilities
  • Provide outreach services to individuals in need, building trust and establishing relationships to promote positive change.
  • Conduct thorough assessments to identify clients' needs and develop personalized plans for reintegration.
  • Assist clients in obtaining necessary documents and resources to secure housing and achieve stability.
  • Negotiate with the criminal justice system to advocate for diversion and alternative sentencing when appropriate.
  • Adhere to contractual obligations and represent Aspire Health Partners in community planning and committees.
  • Participate in required trainings and meetings to stay up-to-date on best practices and industry developments.
Requirements
  • Ability to maintain a Level II Background clearance.
  • Acceptable Motor Vehicle Registration driver's license record.
  • High School Diploma required; Bachelor's Degree preferred.
  • 1-2 years of experience working with targeted populations may substitute for higher education.
