Job Summary:

We are seeking a dedicated and compassionate Registered Nurse to provide skilled nursing care in patients' homes. As a member of our team at CenterWell Home Health, you will be responsible for admission, case management, ongoing care, and coordination of healthcare personnel. This role requires strong med surg, ICU, ER, or acute care experience, as well as a valid driver's license and reliable transportation.

Responsibilities:

* Provide skilled nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care.
* Develop and implement individualized care plans to meet the unique needs of each patient.
* Coordinate with healthcare personnel, including physicians, therapists, and other healthcare professionals, to ensure seamless care delivery.
* Maintain accurate and up-to-date records of patient care, including progress notes, medication lists, and other relevant information.

Requirements:

* Current RN licensure in the state of practice.
* Strong med surg, ICU, ER, or acute care experience, with a focus on home health care.
* Valid driver's license and reliable transportation.
* Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
