Your Role

As the Lead Care Coordinator, you will report to the market clinical leader and oversee the Care Coordinator team, ensuring exceptional support for patients enrolled in our kidney hero program. Your responsibilities will include training staff, conducting quality audits, and collaborating closely with the team to enhance patient care across the continuum. This role combines leadership with direct patient interaction, allowing you to coordinate resources, identify care gaps, and provide follow-up support.

Key Responsibilities
  • Supervise daily operations of the Care Coordinator team under the guidance of senior management.
  • Engage in employee performance evaluations and assist in the hiring and onboarding processes.
  • Mentor and monitor Care Coordinator staff to ensure ongoing development and compliance with quality standards.
  • Conduct chart audits and ensure training programs meet the needs of the team.
  • Communicate with patients to assess their clinical needs and connect them with appropriate resources.
  • Coordinate with healthcare providers to facilitate patient appointments and follow-up care.
  • Maintain up-to-date patient records and provide educational materials to patients and their families.
  • Foster strong relationships with patients throughout their healthcare journey, ensuring a foundation of respect and empathy.
  • Collaborate with the clinical team to manage care transitions and address any gaps in service.
  • Serve as a subject matter expert on care coordination workflows within the market.
Qualifications
  • High school diploma or equivalent required.
  • Minimum of 2 years of experience in a healthcare office environment.
  • At least 2 years of customer service experience.
  • Current Basic Life Support (BLS) certification required.
  • 1+ years of experience in a leadership role.
Preferred Qualifications
  • Current, unrestricted LPN/LVN license in the state of practice.
  • Intermediate proficiency in Microsoft Office Suite.
  • Strong verbal and written communication skills.
  • Community Health Worker Certification (CHW) is a plus.
  • Ability to handle confidential information with discretion.
