Your Responsibilities

As the Lead Care Coordinator, reporting to the market clinical leader, you will oversee the Care Coordinator team supporting patients enrolled in our kidney hero program. Your role will involve training and quality assurance to uphold our standards. You will work closely with the team to develop, implement, and refine Care Coordinator protocols, ensuring safe and effective patient assistance throughout their care continuum. This position requires direct patient interaction to coordinate resources, identify care gaps, and provide timely follow-ups.

Daily Activities
  • Oversee daily operations of the Care Coordinator team under the guidance of senior management.
  • Engage in employee performance evaluations and assist in the hiring and onboarding processes.
  • Provide ongoing mentorship and support to Care Coordinator staff.
  • Conduct chart audits in accordance with established policies.
  • Make outbound calls to patients to assess their clinical needs and connect them with community resources.
  • Facilitate appointments by coordinating with healthcare providers.
  • Respond to inbound inquiries from patients and providers.
  • Ensure comprehensive follow-up with patients to address their needs and schedule future interactions.
  • Collaborate with clinical resources to ensure seamless patient care transitions.
  • Monitor patient hospitalizations and coordinate necessary follow-ups.
  • Maintain up-to-date patient records and provide educational materials.
  • Assist in the referral process as required.
  • Support patients and families in navigating the healthcare system.
  • Build trusting relationships with patients throughout their healthcare journey.
  • Work closely with the clinical team to manage care-related logistics.
  • Serve patients in various settings, including in-person and virtual visits.
  • May act as a preceptor, managing the preceptor program.
  • Serve as a subject matter expert on care coordinator workflows.
Qualifications
  • High school diploma or equivalent is required.
  • Minimum of 2 years of experience in a healthcare office environment.
  • At least 2 years of customer service experience.
  • Current Basic Life Support (BLS) certification is required.
  • 1+ years of experience in a leadership role.
Preferred Qualifications
  • Current, unrestricted LPN/LVN license with community health worker experience.
  • Proficiency in MS Office applications.
  • Strong verbal and written communication skills.
  • Community Health Worker Certification (CHW) is a plus.
  • Ability to handle confidential information with discretion.
