Job Summary

We are seeking a highly skilled Community Integration Worker to join our team at Ascentria Care Alliance. As a Community Integration Worker, you will play a vital role in identifying, assessing, and referring individuals to services that meet their unique needs.

Key Responsibilities
  • Provide direct support, advocacy, and oversight to individuals served, ensuring their treatment plans are implemented effectively.
  • Assess and evaluate individual needs, developing a plan of care that addresses their specific requirements.
  • Establish connections within the community, making referrals, coordinating, and advocating for access to services and supports identified by the individual.
  • Build strong relationships with external and internal stakeholders, including public/private guardians, provider agencies, and community vendors.
  • Conduct job responsibilities in a way that respects the rights, privacy, and dignity of the people served.
  • Ensure documentation standards, site standards, and services delivered meet and/or exceed contractual, regulatory, and agency obligations.
  • Meet all documentation deadlines, including ISP, assessments, census, continuing stay reviews, etc.
Requirements
  • High School Diploma or equivalent required; related associate's degree preferred.
  • 6 months of related experience desired.
  • Valid State-issued Driver's License and proof of current insurance required.
  • MHRT-C or MHRT-C Provisional certification and maintenance with 18 approved CEUs every 2 years.
