OVERVIEW:


Deliver empathetic support to individuals affected by HIV/AIDS and their families, fostering greater involvement in their health management.


KEY RESPONSIBILITIES:
Demonstrate a willingness to accept guidance from supervisors.


Collaborate effectively with primary care teams and other personnel to identify newly diagnosed patients or those re-engaging with care who would benefit from peer support.


Engage with newly diagnosed individuals and those transitioning back into care to provide encouragement, illustrating that living well with the condition is achievable.

Exhibit strong problem-solving skills and manage crises appropriately, including knowing when to seek clinical input.

Provide health education and follow-up support to new clients after they have been assigned a case manager.

Assist clients in navigating the healthcare system, adhering to medication schedules, making lifestyle adjustments, and accessing community resources.


In accordance with Ryan White Program guidelines, enhance client engagement and adherence by conducting appointment reminders, coordinating transportation to appointments, and more.

Document peer interactions (i.e., in-person or via phone).

Complete and maintain necessary documentation and forms.

Uphold confidentiality and ethical standards at all times.

Create and nurture a welcoming, client-focused environment.

Proactively seek and suggest improvements for the program.

Work alongside staff to design and implement client-centered support initiatives.

Participate in relevant meetings, including community gatherings on behalf of CareSouth and/or the clients.

Co-facilitate and organize CareSouth's RW monthly Consumer Advisor Board Meeting alongside fellow Patient Support Specialists or designated personnel.

Arrange transportation for RW clients to and from meetings and healthcare appointments.

Encourage adherence to medical treatments and increased engagement in self-care by accompanying clients to medical visits.

Promote community involvement.

Engage in community outreach initiatives.


Adhere to all policies and procedures related to time and attendance management (clocking in & out, reviewing timecards, requesting edits, etc.).

Report any significant issues to supervisors promptly.

Comply with training requirements and submit documentation to Human Resources and the Ryan White Program Manager or their designee for record-keeping.

MINIMUM QUALIFICATIONS:
A high school diploma or equivalent is preferred.


Must be a former or current client of Ryan White or eligible for Ryan White services and willing to share personal HIV status and experiences with clients.

At least one year of experience in this or a related field is required.

Ability to work independently while seeking guidance when necessary.

Strong interpersonal skills to connect with diverse groups and individuals at all levels.

Capability to engage with a multicultural community (including various ethnicities, sexual orientations, and socioeconomic backgrounds).

Ability to adhere to the required work schedule.

Proficient writing, verbal communication, telephone, and time management skills.

Proficiency in using computers and various software applications.

SUPERVISION:
No supervisory responsibilities.

WORKING CONDITIONS:
Visual acuity - always required.

Hand-eye coordination - consistently necessary.

Primarily sedentary work; extensive time may be spent using a computer and management information systems.

May involve climbing stairs in various office environments.

Occasional lifting, pushing, or pulling of up to 50 lbs.

Some stooping, bending, and sitting may be required.

Frequent walking over short distances.

Indoor, climate-controlled environment with some exposure to outdoor elements.

Potential exposure to diseases or infections.

No vibrations present.

Exposure to both artificial and natural light.
