About the Role

We are seeking a highly skilled Care Coordinator to join our team at UnityPoint Health. As a Care Coordinator, you will play a critical role in providing whole-person care coordination across medical, behavioral, and social service community systems.

Key Responsibilities
  • Develop and implement care coordination plans to address the needs of adults with Serious Mental Illness (SMI) or Children with Serious Emotional Disturbance (SED).
  • Respond to treatment barriers and refer high-risk patients to Nurse Care Manager.
  • Educate members on health information, referrals to resources, and communicate with other health and social service providers.
  • Promote patient health and ensure that all personal health goals are included in person-centered care management plans.
  • Conduct home visits, contact members by phone, attend meetings, and document services using the Center's Electronic Health Record.
Requirements
  • Bachelor's Degree in a Human Service-related field.
  • Valid driver's license.
  • Ability to write, read, comprehend, and speak fluent English.
Preferred Qualifications
  • 1 year of experience working with adults and/or children with mental illness.
  • Ability to form good working relationships with clients.
  • Knowledge of community resources.
