Key Responsibilities- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare providers to develop and implement comprehensive care plans that address the unique needs of each individual.
- Address Treatment Barriers: Identify and address treatment barriers for patients, including access to healthcare services, medication adherence, and social determinants of health.
- Provide Education and Support: Educate patients and families on health information, referral to resources, and other support services to promote healthy behaviors and outcomes.
- Coordinate Services: Communicate and coordinate services with other healthcare providers, social service agencies, and community resources to ensure seamless care transitions and optimal patient outcomes.
- Document Services: Accurately document services provided, including health assessments, care plans, and progress notes, using the Center's Electronic Health Record and other required systems.
- Participate in Quality Improvement: Contribute to continuous quality improvement activities, including data collection, tracking, and analysis, to ensure high-quality care and services.
- Bachelor's Degree in a Human Service-related Field: A degree in a field such as social work, psychology, or nursing is required.
- Valid Driver's License: A valid driver's license is required for travel to patient homes and other locations.
- Experience Working with Adults and/or Children with Mental Illness: Experience working with adults and/or children with mental illness is preferred.
- Ability to Form Good Working Relationships: The ability to form good working relationships with patients, families, and healthcare providers is essential.
- Knowledge of Community Resources: Knowledge of community resources and services is preferred.
