Key Responsibilities- Develop and Implement Care Plans: Collaborate with patients, families, and healthcare providers to develop and implement comprehensive care plans that address the unique needs of each individual.
- Coordinate Services: Facilitate communication and coordination between healthcare providers, social services, and community resources to ensure seamless care delivery.
- Provide Education and Support: Educate patients and families on health information, resources, and services to empower them to take an active role in their care.
- Monitor and Evaluate Progress: Regularly assess patient progress, identify areas for improvement, and make adjustments to care plans as needed.
- Education: Bachelor's degree in a human service-related field.
- Experience: 1 year of experience working with adults and/or children with mental illness, preferably in a healthcare or social services setting.
- Skills: Excellent communication and interpersonal skills, ability to form good working relationships with clients, and knowledge of community resources.
