Job Title: Resident Assessment Manager

Location: Memorial City Nursing and Rehabilitation Center - Houston, TX

Estimated Salary: $68,000 - $90,000 per year

The Regency Integrated Health Services team is seeking a seasoned Resident Assessment Manager to join our interdisciplinary team at Memorial City Nursing and Rehabilitation Center in Houston, TX. As a key member of our care coordination team, you will be responsible for ensuring accurate and timely completion of resident assessments, collaborating with the facility Director of Rehab to optimize assessment reference dates, and tracking Skilled customers to determine continued and appropriate Medicare/Managed Care eligibility and benefit period. With a strong background in healthcare and experience with MDS completion, reimbursement, clinical resource utilization, and case management, you will thrive in this role.

Key Responsibilities:
  • Ensure accurate and timely completion of resident assessments.
  • Collaborate with facility Director of Rehab to optimize assessment reference dates.
  • Track Skilled customers to determine continued and appropriate Medicare/Managed Care eligibility and benefit period.
  • Communicate regularly with Regional Care Management Specialist to discuss identified clinical reimbursement issues.

Required Skills and Qualifications:
  • Graduate of an accredited school of nursing with current RN or LVN licensure.
  • Minimum 2 years of healthcare experience, preferably with MDS completion, reimbursement, clinical resource utilization, and case management.
  • Strong analytical and problem-solving skills, with ability to efficiently lead interdisciplinary teams and coordinate RAI process.
