Position Overview:

Key Responsibilities:
  • Exhibits sound clinical judgment and decision-making skills when prioritizing patient care.
  • Manages medication refills with necessary provider authorization.
  • Maintains detailed and accurate documentation of patient histories, assessments, care plans, and treatments.
  • Participates in the onboarding, training, and mentorship of new staff members.
  • Identifies and addresses the safety needs of the patient population.
  • Engages with patients and families in a respectful and professional manner, ensuring their needs and concerns are addressed.
  • Fosters a collaborative team environment, maintaining a positive attitude and openness to new ideas.
  • Actively engages in care transitions, recalls, high-risk management, and vaccination protocols.
  • Adheres to established protocols for managing chronic and acute patient conditions.
  • Contributes to departmental meetings and initiatives.
Additional Responsibilities:
  • Complies with all Standards of Conduct and relevant policies.
  • Ensures the confidentiality of patient and employee information.
  • Adheres to safety protocols and procedures.
Qualifications:
  • Graduate of an accredited nursing program with licensure as a Registered Nurse.
  • Basic Life Support (BLS) certification required; bilingual skills preferred.
  • Minimum of one year of relevant nursing experience in a community health or outpatient care setting preferred.
Skills and Competencies:
  • Strong written and verbal communication skills.
  • Proficient in using personal computers in a Windows environment.
