Position Overview

Northwestern Memorial Healthcare is seeking a dedicated Nurse Practitioner or Physician Assistant to join our outpatient NMH Transitional Care Clinic.

The mission of the NMH Transitional Care Clinic is to enhance accessibility and improve health outcomes for vulnerable patients facing complex medical and psychosocial challenges. We employ an integrated, interdisciplinary team-based approach to deliver high-quality, compassionate transitional care tailored to the needs of our patients, healthcare providers, and community partners. The Transitional Care Clinic focuses on patients following an acute care crisis, assisting them in improving their health, navigating the healthcare system, and transitioning smoothly to long-term primary care providers in their communities.

Work Schedule: Monday to Friday, daytime hours; no night, weekend, or holiday shifts required.

On-call duties are necessary, approximately 9 weeks per year, with MDs and APPs rotating through a pager call coverage system. This does not require in-person attendance but involves forwarding clinic pages to the assigned individual's personal pager for any calls that may occur overnight or on weekends.

Key Responsibilities:

- Demonstrate clinical expertise and extensive knowledge through patient care activities, including diagnosing and managing a diverse range of health issues.
- Be accountable for the health outcomes of a designated patient population.
- Collaborate with physicians and other healthcare professionals in your area of clinical expertise.
- Develop educational resources for patients, families, and staff in your specialty.
- Mentor graduate students in advanced practice and patient care.
- Actively engage in quality improvement initiatives and/or research relevant to your field.

Essential Duties:

- Conduct thorough assessments of patients' health by obtaining health histories and performing physical examinations.
- Provide direct clinical care to patients.
- Prescribe medications and therapies; order and interpret laboratory tests, x-rays, and procedures as per clinical privileges.
- Evaluate patients to determine their need for additional healthcare services.
- Offer comprehensive education to patients and their families.
- Document all care and education provided in the medical record, including procedures performed.
- Refer patients to other disciplines as necessary.
- Serve as a consultant to other disciplines in your area of expertise.
- Integrate research-based knowledge and clinical expertise in decision-making processes.
- Maintain a professional image in appearance and conduct.
- Advocate for patients, families, colleagues, and the community.
- Keep certifications and continuing education up to date as required for licensure and privileging.
- Participate in departmental committees/task forces as appropriate.
- Develop and implement educational strategies for patients and their families.
- Contribute to programs addressing the continuing education needs of staff.
- Serve as a role model and preceptor for students.
- Provide formal education to both internal and external groups related to your specialty.
- Engage in quality improvement and/or research initiatives for specific patient populations as needed.

Qualifications

Required:
  • Master's degree from an accredited nursing school or a master's degree from an accredited Physician Assistant Program.
  • Current licensure as a Registered Nurse in Illinois and as an Advanced Practice Registered Nurse in Illinois. Certification as an Advanced Practice Registered Nurse by a national certifying body approved by the Illinois Department of Regulation APRN Board. Eligible to apply for a controlled substance license and DEA number upon licensure.
  • Privileged and credentialed through the NMH Medical Staff Office (MSO).
  • Current Basic Life Support (BLS) certification issued by the American Heart Association.
  • Minimum of one (1) year of experience as an Advanced Practice Provider.
