The RN plays a vital role within the primary care team and reports directly to the Clinical Services Manager (CSM). This position supports the Advanced Practice Clinician (APC) and our members in coordinating care services. The RN collaborates closely with all team members and may assist multiple providers. Under the guidance of the provider, the RN undertakes activities delegated by the provider or the team CSM. The RN is knowledgeable about and supports the Care Program Models including Institutional Special Needs Plan (ISNP), Institutional Special Needs Plan (IESNP), and Transitions to Skilled (TTS), working alongside the team to achieve program goals. This role will assist our members both telephonically and in-person within patient and home settings.

Key Responsibilities:

  • Builds and maintains effective communication and a trusting collaborative relationship with the Integrated Care Team (ICT), member/Responsible Party (RP), and primary caregiver staff in the Assisted Living Community (ALC) and Advanced Practice Clinic (APC).
  • Coordinates with various organizational units for seamless benefit coordination, including utilization review staff, provider relations liaison, and pharmacy.
  • Monitors member progress towards goals and adjusts the Individual Care Plan (ICP) as needed.
  • Handles administrative tasks for or on behalf of the Primary Care Provider (PCP) and/or APC.
  • Engages in program effectiveness analysis and strategic planning.
  • Conducts member visits and rounding as directed by the APC.
  • Updates the APC on member status for evaluation and ICP modifications.
  • Communicates with members, families/RPs, primary caregivers, and other ICT members during each visit.
  • Assists with clinical and quality responsibilities for members, such as addressing gaps in care.
  • Represents the APC in meetings and communicates ICP updates to the ICT.
  • Delivers clinical educational programs for facility staff.
  • Conveys information on behalf of the APC to members, families/RPs, and primary caregiver staff at the ALC.
  • Coordinates and collaborates with health plans/clients.
  • Assists with Advance Care Planning (ACP).
  • Collects clinical data including functional status, BMI, vital signs, and other relevant information, entering results into the Electronic Medical Record (EMR) for APC review.
  • Facilitates care coordination activities within the ALC to ensure member needs are met, including external provider appointments, therapy needs, durable medical equipment (DME), and transportation.
  • Stays informed about all Model of Care (MOC), documentation, and policy updates communicated through emails, newsletters, and team meetings.
  • Utilizes clinical resources independently, including but not limited to Resource1, iCoreCodeGenius, Sparq, Learnsource, and other internal Optum sites.
  • Maintains confidentiality of health information in accordance with state and federal regulations, including HIPAA.

Required Qualifications:

  • Current unrestricted Registered Nurse license in Oregon.
  • 3+ years of RN experience.
  • 1+ years of case management experience.
  • Valid driver's license and ability to travel locally 75% of the time.
  • Experience with electronic medical records.
  • Basic Microsoft Office skills and proficiency in a Windows environment.
  • Demonstrated excellent communication and customer service skills.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN).
  • Experience in post-acute care settings, such as long-term care or hospice, and/or case management experience as an RN.
  • Experience working with the geriatric population.
  • Familiarity with the long-term care business environment.
