Key Responsibilities:
- Conduct comprehensive assessments to identify post-acute care needs.
- Develop and implement transition plans to ensure seamless patient care.
- Coordinate care among healthcare providers and community resources.
- Provide education to patients and families regarding care plans and available resources.
- Collaborate with the care team to monitor and evaluate the effectiveness of care management strategies.
- Reassess the need for care management services as necessary.

Qualifications:
- A nursing degree or diploma is required upon hire.
- A Bachelor’s degree in Nursing or higher is expected within three years of employment.
- An active Oregon Registered Nurse License is mandatory upon hire.
- A minimum of two years of experience in an acute care hospital or community-based setting is essential.
- At least one year of experience with electronic medical records or documentation systems is required.
- Experience in home health, mental health, substance use, hospice, utilization review, discharge planning, or care management is preferred.

Preferred Qualifications:
- A Bachelor’s degree in nursing at the time of hire is advantageous.
- Certification in Care Management (CCM) is preferred upon hire.
