Job Title: Housing Focused Case Manager Team Lead

We are seeking a compassionate and empathetic individual to join our team as a Housing Focused Case Manager Team Lead. As a key member of our Supportive Services team, you will provide trauma-informed, guest-driven case management services to individuals experiencing homelessness, with the goal of helping them achieve stable housing solutions quickly and efficiently.

Key Responsibilities:
  • Provide direct, trauma-informed case management services to guests, including crisis intervention, harm reduction, outreach, ongoing assessments, goal setting, and creative problem solving.
  • Support a small team of Case Managers through client interactions, administrative needs, and other duties, including maintaining professional boundaries, emphasizing teamwork, and creating an environment that fosters growth.
  • Establish an environment that promotes growth, support, and advocacy for both shelter guests and team members.
  • Assign guests to caseloads within 7 days of arrival to shelter, track initial case plans, case noting, and case plan reviews, and report to the Supportive Services Manager to ensure adherence to OHS standards.
  • Take a lead role in community partnerships, assist with bed management/prioritization, and collaborate with the Supportive Services Manager and Team Lead to support Services team.
Requirements:
  • Lived, work, or volunteer experience preferred or a degree in a related field.
  • Experience working or volunteering with diverse populations.
  • Excellent writing and documentation skills.
  • Ability to work with community partners, self-start, and self-direct.
  • Ability to accept direction from Supervisor, excellent organizational skills, and strong computer skills.
  • Familiarity with the Housing First philosophy and demonstrated awareness of and sensitivity to diverse populations.
