Care Manager Job Summary
We are seeking a skilled Care Manager to join our team at Centene. As a Care Manager, you will play a critical role in developing and implementing personalized care plans for our members. Your expertise in nursing and care management will enable you to assess the needs of our members and provide high-quality, cost-effective healthcare outcomes.
Key Responsibilities:
- Evaluate the needs of our members and identify barriers to accessing care
- Develop and implement care plans that address the unique needs of each member
- Collaborate with healthcare providers to ensure timely access to needed care or services
- Provide education and support to members and their families on disease processes, healthcare options, and benefits
- Monitor member status and progress towards care plan goals
Requirements:
- RN - Registered Nurse - State Licensure and/or Compact State Licensure required
- 2 – 4 years of related experience in nursing and care management
