Job Title: Care Manager

Job Summary:

We are seeking a skilled Care Manager to join our team at Fortcom Health Services. As a Care Manager, you will be responsible for conducting initial and ongoing psychosocial evaluations of our adult foster care program members. You will also select, train, evaluate, and supervise AFC caregivers in conjunction with our registered nurses.

Responsibilities:

  • Conduct psychosocial evaluations of AFC program members
  • Select, train, and supervise AFC caregivers
  • Participate in the development and implementation of AFC plans of care
  • Conduct on-site visits with AFC program members
  • Review AFC caregiver logs
  • Report changes in AFC program members' conditions to their registered nurses
  • Participate in discharge and transition planning

Requirements:

  • Bachelor's degree and two years of clinical experience in elder care
  • Social worker license from the Massachusetts Board of Registration
