We are seeking a highly skilled Clinical Care Provider to join our team at HexaQuEST Health, Inc. As an Advanced Practice Healthcare Professional, you will be responsible for providing high-quality patient care and management in our clinics.
The ideal candidate will have a strong background in nursing or medicine, with experience in assessment, diagnosis, treatment, and follow-up care. You will work closely with our team to develop and implement patient care plans, utilizing your knowledge and skills to provide exceptional care.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Conduct thorough physical exams and gather relevant data
- Diagnose and treat medical conditions, as well as refer patients to specialists when necessary
- Collaborate with healthcare teams to ensure seamless patient care
Requirements:
- Graduate of a nurse practitioner program or physician assistant program
- Current licensure in California
- Excellent communication and interpersonal skills
