Nurse Care Manager

Job Summary:
We are seeking a skilled Nurse Care Manager to join our team at Fortcom Health Services. As a key member of our Adult Family/Foster Care program, you will be responsible for providing high-quality nursing care to our members.
Responsibilities:
- Conduct monthly home visits to assess the health status of our members.
- Develop and review individualized care plans to ensure the best possible outcomes.
- Train and supervise AFC caregivers to ensure they have the necessary skills to provide quality care.
- Monitor the health status of our members and report any changes to their physicians.
- Complete nursing progress notes and submit semiannual health-status reports to primary care physicians.
- Plan and implement discharges from the AFC program as needed.
- Provide ongoing training to AFC caregivers on health and aging.
Requirements:
- Demonstrated ability to coach, train, and provide staff development.
- Proficiency in Microsoft Word, Excel, and PowerPoint.
- Associate or Bachelor's degree in Nursing and current RN licensure in Massachusetts.
- At least two years of previous home health care experience.
- Bilingual (Spanish or Haitian Creole) is a plus.
