We are seeking a highly skilled and experienced Nurse Manager to lead our Adult Foster Care team at Fortcom Health Services. The successful candidate will be responsible for managing and supporting our Adult Family/Foster and Home Health Care services to ensure our programs run smoothly.
Key Responsibilities- Supervise and manage a team of Home Visit RNs
- Complete MDS-HC and other clinical assessments
- Develop and review AFC plans of care
- Train, evaluate, and supervise AFC caregivers
- Monitor health status and report changes to physicians
- Review AFC caregiver logs and conduct admission on-site visits
- Submit semiannual health-status reports to primary care physicians
- Plan and implement discharges from the AFC program
- Provide ongoing training to AFC caregivers
- MS or Bachelor's degree in Nursing
- Full licensure as a RN by the Mass Board of Registration
- At least five years of previous home health care experience
- Proficiency in Microsoft Office (Word, Excel, PowerPoint)
