Nurse Care Manager

Fortcom Health Services is seeking a skilled RN to join our team as a Nurse Care Manager for our Adult Family/Foster Care program. The ideal candidate will have experience in home health care and be able to develop and implement individualized care plans for our members.
Key Responsibilities:
Conducting regular home visits to assess the health and well-being of our members
Developing and reviewing care plans to ensure the best possible outcomes for our members
Providing training and support to caregivers to help them provide high-quality care
Monitoring the health status of our members and reporting any changes to their physicians
Maintaining accurate records of care provided and outcomes achieved
Collaborating with our Program Directors and President to ensure the smooth operation of our program
Requirements:
Associate or Bachelor's degree in Nursing
Current RN licensure in Massachusetts
At least two years of experience in home health care
Strong communication and interpersonal skills
Ability to work independently and as part of a team
Bilingual skills (Spanish or Haitian Creole) are a plus
Flexible work arrangements available.
