Schedule:
Weekdays and weekends, 8:00 AM - 5:00 PM; PRN; must be able to train Monday - Friday for at least 2 weeks of orientation.

The RN Case Manager plays a pivotal role in the intricate discharge planning process and is a key contributor to the healthcare team.

This position involves collaboration with physicians and other healthcare professionals to assess, plan, implement, and evaluate comprehensive and safe discharge strategies.

The RN Case Manager will work closely with the MSW to navigate psychosocially complex cases. Additionally, this role is responsible for ensuring effective care coordination.

In conjunction with the healthcare team, the RN Case Manager accepts responsibility, authority, and accountability for the management of patient care.

This is in alignment with the current policies, procedures, mission, vision, and values of Novant Health. The RN Case Manager serves as a resource for discharge planning and care coordination for patients with complex medical needs. It is essential to maintain up-to-date knowledge of available community resources and post-acute care options.

Qualifications:
Education:
Associates Degree in Nursing or BSN, required. 4 Year / Bachelor's Degree, preferred.

Experience:
Two or more years of healthcare experience, preferred. Experience in Case Management or a related field is preferred.

Licensure/Certification:
Current RN licensure in the state of employment, required.

Additional Skills Required:
Availability for weekend and holiday coverage.

Additional Skills Preferred:
Proficiency in Microsoft Office products and keyboarding skills.
