Compensation:
Pay Rate: $22.00 per hour
Role Overview:
The purpose of this position is to deliver personalized support to HOPE clients, addressing personal and social challenges that may hinder their success in community environments. This role may also involve providing on-site training and intervention for clients placed in community employment settings, working collaboratively with clients and employers to establish job training objectives and ensure ongoing success.
Key Responsibilities:
- Facilitate necessary support to ensure client success in employment placements, including evaluation, training, goal setting, and follow-up services for job retention.
- Serve as a liaison with employers, offering training and support, and consulting on employment-related matters to promote long-term employment success.
- Monitor assigned clients' programs, providing advocacy and referrals as needed, and assisting with accommodations and ADA-related issues.
- Adhere to employer policies and procedures while on-site.
- Represent the agency within the community, maintaining professional relationships with family members, employers, and funding sources.
- Complete all required documentation and maintain accurate case files, conducting regular meetings and preparing individualized service plans.
- Ensure compliance with agency and departmental safety and confidentiality standards.
- Work independently in various community settings, developing and implementing instructional plans and social skills training as needed.
- Adapt to changing needs and requests from contracting employers, including work hours and locations.
- Conduct task analysis and time studies for wage evaluations as mandated.
- Act as a mandated reporter for abuse.
- Perform additional duties as assigned to enhance program efficiency.
Qualifications:
High School diploma, G.E.D., or equivalent, along with one year of relevant experience. A clear driving record and a valid Driver's License are required.

- Ability to connect with individuals with disabilities, their families, and community members.
- Flexibility in work hours, locations, and assignment types.
- Strong observational, evaluative, and communication skills, both verbal and written.
- Capacity to work collaboratively as part of a team while accepting supervision.
- Ability to teach effective work habits and social skills in diverse settings.
- Critical thinking skills for decision-making, along with good organizational and record-keeping abilities.
- Basic computer proficiency.
- Ability to transport clients to various employer locations as necessary.
Physical and Environmental Conditions:
This role involves significant driving and transporting clients within the community, as well as standing and walking. Exposure to various environmental conditions may occur, including dust, noise, and weather variations.
