SUMMARY

Under the supervision of the Access Manager, the Registered Nurse acts as a care coordinator for designated patients. This role involves assessing and addressing patient and family needs while delivering preventive and supportive care to those facing terminal illnesses. The hospice nurse employs assessment, intervention, and educational skills to support the patient and family unit.

KEY RESPONSIBILITIES
  1. Conducts admission processes across all Alive Hospice services.
  2. Performs comprehensive physical, psychosocial, and spiritual evaluations of patients and their families.
  3. Utilizes the nursing process to develop and implement a hospice care plan in partnership with interdisciplinary team members.
  4. Proactively identifies and initiates interventions to prevent complications and alleviate symptoms, ensuring optimal well-being.
  5. Delivers skilled nursing care to patients.
  6. Educates patients and caregivers about the care plan, its objectives, and the implications of diagnoses and prognoses.
  7. Provides guidance and training to patients and caregivers on resource conservation techniques to support home care.
  8. Acts as a liaison between Alive Hospice and the patient's primary healthcare provider.
  9. Assesses the need for and organizes home equipment, medications, and supplies in accordance with Alive Hospice policies.
  10. Reports any unusual or concerning patient or caregiver issues to the Access Manager and relevant team members.
  11. Instructs, supervises, and mentors CNT/HHA and other staff as designated by the Access Manager.
  12. Attends to patient deaths, following Alive Hospice protocols, and offers support to caregivers.
  13. As needed, provides coverage for after-hours care or staffing emergencies.
  14. Completes all clinical documentation promptly as per Alive Hospice policy.
  15. Participates in interdisciplinary care planning conferences and team meetings as required.
  16. Collaborates with team members to ensure effective, empathetic support and care for patients and caregivers.
  17. Notifies appropriate staff of any changes in patient status following Alive Hospice procedures.
  18. Functions as a case manager within the patient care team, working with others to achieve desired patient outcomes.
  19. Demonstrates effective resource management while considering the financial implications related to the agency's budget.
  20. Communicates with physicians, medical directors, and other healthcare providers regarding patient assessments and responses to care plans.
  21. Serves as a clinical mentor for professional students as requested.
  22. Supports the agency's commitment to providing educational and research opportunities to enhance the field of hospice and palliative care.
  23. Participates in weekend and after-hours admissions as assigned.
  24. Other duties may also be assigned.
SUPERVISORY ROLE

Provides oversight for CNT/HHA in home care settings, adhering to the organization's policies and applicable regulations.

LANGUAGE COMPETENCIES

Ability to read, analyze, interpret, and apply theories and concepts from professional clinical literature. Proficient in writing documentation that accurately reflects assessment, planning, implementation, and evaluation of care. Capable of drafting policies and professional correspondence for various healthcare entities. Effectively communicates, educates, and builds rapport with patients, families, and other healthcare professionals.

MATHEMATICAL ABILITIES

Ability to engage with mathematical concepts such as dosage calculations, probability, and basic statistical inference. Proficient in applying concepts like fractions, percentages, and ratios to patient care and healthcare management challenges. Capable of interpreting laboratory and diagnostic data.

REASONING SKILLS

Demonstrates critical thinking abilities. Applies theoretical knowledge from professional clinical training and continuing education to practical situations. Assesses patient and family status and responds with appropriate interventions. Prioritizes care based on patient needs and provides effective crisis intervention. Capable of following complex care directions and delegating, teaching, and monitoring care of peers and support staff. Formulates hypotheses and utilizes judgment to solve problems. Applies assessment, planning, implementation, and evaluation skills to meet care requirements.

CERTIFICATIONS AND LICENSES

Current RN License in Tennessee is required. Hospice certification is preferred. If driving is necessary for job duties, a valid driver's license and automobile insurance as mandated by Tennessee State Law are required.

Requirements

CONTINUING EDUCATION

This position requires the completion of 15 hours of continuing education annually, covering topics that enhance the ability to fulfill the responsibilities outlined above. Regulatory agencies may require additional hours for certain disciplines to maintain licensure or certification.

EDUCATION AND EXPERIENCE

Registered Nurse with a current nursing license; BSN preferred. Ability to apply for a Tennessee license within 30 days of hire. Knowledge of or willingness to learn about hospice care is essential. A minimum of one year of nursing practice or equivalent experience is required. Experience in hospice, oncology, home health, or long-term care is preferred.
