Position Summary

The Hospice Clinical Liaison plays a pivotal role in fostering hospital-based outreach, managing referrals, and enhancing admission rates for hospice services.

Key Responsibilities
  1. Collaborate with clinical leadership to identify and cultivate potential referral sources while maintaining strong professional relationships with existing providers to enhance the understanding and utilization of hospice services.
  2. Exhibit exceptional customer service and critical thinking abilities consistently.
  3. Work closely with the medical community, patients, and families to facilitate eligible hospice admissions.
  4. Engage with referral partners within the hospital to streamline admissions for patients qualifying for GIP level of care.
  5. Oversee patient discharge processes from the hospital and manage transitions to home, inpatient units, or long-term care facilities.
  6. Track referral sources and assess the utilization of hospice services, preparing activity reports on relationship management.
  7. Maintain and manage relationships with skilled nursing and long-term care facilities.
  8. Work independently with minimal supervision.
  9. Coordinate educational presentations and in-service training sessions.
  10. Demonstrate effective resource management while being mindful of budgetary implications.
  11. Keep a log of referral source expenditures.
  12. Document interactions and activities in the EMR and CRM systems.
  13. Conduct on-site assessments to identify needs and provide support to prospective patients and referral sources.
  14. Communicate effectively with physicians and healthcare providers regarding patient evaluations and care plans.
  15. Complete necessary admission documentation within the scope of practice.
  16. Facilitate appropriate interventions during complex referral and admission processes.
  17. Perform additional duties as assigned.
Supervisory Responsibilities

This position does not include administrative supervisory responsibilities over agency staff.

Continuing Education Requirements

Employees are required to complete 15 hours of continuing education annually, focusing on topics that enhance their ability to fulfill the responsibilities of this role. Some disciplines may require additional hours for licensing or certification.

Qualifications

To be successful in this role, candidates must meet the following qualifications:

Education and Experience

Registered Nurse with a minimum of five years of experience in healthcare, preferably in hospice sales and marketing.

Licenses and Certifications

Must possess a current Tennessee or compact RN license, along with a valid Tennessee driver's license and automobile insurance as mandated by state law. This role may involve frequent use of an automobile.
