Public Health Management Corporation is seeking a highly skilled Family Nurse Practitioner to join our team. As a key member of our Primary Care Medical Home team, you will provide high-quality primary and urgent care services to diverse and underserved populations.
Key Responsibilities- Provide comprehensive primary care services, including health screenings, immunizations, and disease prevention programs.
- Manage acute and chronic health problems using evidence-based protocols and collaborate with community health nurses on care and follow-up.
- Advocate for the role of advanced practice nurses in underserved areas through professional activities and maintain positive relationships with funding sources and community organizations.
- Document all services in a timely manner and supervise staff as appropriate.
- Participate in public health management of communicable diseases in shelters and other health initiatives.
- Current licensure as an RN and APN Nurse Practitioner.
- Extensive knowledge of current health care needs of the homeless and mentally ill adults.
- Ability to engage resistant clients, collect adequate client data, and manage or refer patients as conditions or protocols dictate.
- Good verbal communication and writing skills, as well as computer skills.
- Ability to prioritize and organize workday to meet changing needs of program and clients.
