As a key member of Strive's clinical team, the Advanced Practice Nurse Specialist will leverage their expertise to deliver high-quality nursing care to patients with chronic and end-stage kidney disease. This role involves collaborating with a multidisciplinary team, including social workers, care coordinators, and physicians, to ensure optimal patient care. The Advanced Practice Nurse Specialist will serve as a trusted advocate and clinician, fostering strong relationships with patients while providing exceptional clinical support.
Key Responsibilities
Responsible for patient outcomes and performance metrics for a designated group of CKD and ESKD patients, ensuring alignment with external providers. Conduct thorough patient assessments, including history-taking and physical examinations, and develop comprehensive care plans. Actively participate in clinical rounds and interdisciplinary meetings, presenting patient cases and providing clinical insights. Ensure care plans adhere to best practices for chronic kidney management, including shared decision-making and advanced care planning. Provide care in various settings, including patient homes and through telehealth services. Liaise with dialysis centers and primary care providers to facilitate seamless transitions in patient care. Maintain current licensure and fulfill continuing education requirements. Adhere to Strive's documentation policies and standards. Deliver primary and urgent care to adult and geriatric patients with chronic kidney disease.Qualifications
Master's degree in Nursing or a related field. Minimum of 2 years of experience as a Nurse Practitioner in relevant specialties. Valid state licensure and national board certification. Current DEA license or eligibility to obtain one within 90 days of hire. BLS or CPR certification is required. Reliable transportation for patient visits. Proficient internet connectivity for telehealth services.Preferred Skills
Familiarity with electronic health records (EHR) systems. Wound care certification is a plus. Experience with telehealth technology.About You
