Work Schedule:
Full-Time, Monday through Friday, 8:00 AM - 5:00 PM

Compensation:
$105,000-148,500** Wider range available depending on experience and location

Role Overview:


The Nurse Practitioner at Oak Street Health is dedicated to delivering effective and equitable value-based primary care to adults on Medicare, ensuring their well-being and minimizing hospital visits.

Nurse Practitioners operate independently while collaborating with physicians based on the individual needs of patients.

They foster long-term relationships with patients, establishing trust and helping them connect with their care plans.


Nurse Practitioners work within our outpatient facilities as part of a collaborative care team, which includes a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers.

We collaborate with a network of distinguished specialists and hospitals for specialized and acute care.

As a result, our healthcare providers can concentrate on clinic-based care, including:
primary care, care coordination, and population health.

Key Responsibilities:

  • Deliver outstanding primary care services.
  • Conduct office consultations for both routine and urgent health issues.
  • Perform annual wellness visits and health risk assessments, requiring a comprehensive view of health and meticulous documentation.
  • Coordinate care with other healthcare providers, specialists, testing facilities, and agencies.
  • Lead population health initiatives in collaboration with the Care Team (e.g., ensuring eligible females receive evidence-based breast cancer screenings every two years).
  • Assist the care team with phone triage and outreach efforts.
  • Educate patients regarding their health conditions, care plans, and treatment options.
  • Participate in promotional activities for Oak Street Health.
  • Conduct home visits as necessary.
  • Perform additional duties as assigned.
This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification in at least one of the following specialties:
  • Family Nurse Practitioner
  • Adult-Gerontology Primary Care Nurse Practitioner
  • Adult Nurse Practitioner
  • Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • US Work Authorization.

Preferred Qualifications:

  • Experience in primary care: internal medicine, geriatrics, or family medicine.
  • Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs).
  • A passion for teamwork and collaboration across functions.

  • Bilingual proficiency in relevant areas.
