The role of an Advanced Practice Nurse at Oak Street Health is to deliver high-quality and equitable primary healthcare services to adults enrolled in Medicare, ensuring their well-being and minimizing hospital admissions. Advanced Practice Nurses operate autonomously while also collaborating with physicians based on the individual needs of patients. They foster long-term patient relationships, which cultivates trust and enhances patients' understanding and connection to their healthcare plans.

Advanced Practice Nurses work within our outpatient facilities as part of a collaborative healthcare team that includes a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers. We collaborate with a network of distinguished specialists and hospitals for specialized and acute care, allowing our healthcare professionals to concentrate on clinic-based services: primary care, care coordination, and population health management.

Key Responsibilities:

  • Delivering outstanding primary healthcare services.
  • Conducting office consultations for both routine and urgent health concerns.
  • Performing annual wellness assessments and health risk evaluations, which necessitate a comprehensive understanding of health and a commitment to precise and thorough documentation.
  • Coordinating care with other healthcare providers, specialists, diagnostic facilities, and agencies.
  • Leading population health initiatives in collaboration with the Care Team (e.g., ensuring all eligible women receive their evidence-based breast cancer screenings biennially).
  • Assisting the care team with telephone triage and outreach efforts.
  • Educating patients regarding their health conditions, treatment plans, and care strategies.
  • Engaging in promotional activities for Oak Street Health.
  • Conducting home visits when necessary.
  • Performing additional duties as assigned.

This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program.
  • National certification in at least one of the following specialties:
    • Family Nurse Practitioner
    • Adult-Gerontology Primary Care Nurse Practitioner
    • Adult Nurse Practitioner
    • Gerontological Nurse Practitioner
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • Authorization to work in the United States.
  • A candidate who exemplifies the values of being 'Oaky'.

Preferred Qualifications:

  • Experience in primary care settings such as internal medicine, geriatrics, or family medicine.
  • Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs).
  • A strong commitment to teamwork and a desire to collaborate across various functions.
  • An interest in being part of an innovative model focused on evidence-based population health.
  • Bilingual proficiency in relevant languages.
