Role Overview:


The role of a Nurse Practitioner at Oak Street Health is to deliver effective and equitable value-based primary care to adults on Medicare, ensuring their well-being and minimizing hospital visits.

Nurse Practitioners operate independently while collaborating with physicians based on the individual needs of patients.

Nurse Practitioners foster ongoing care and establish meaningful relationships with patients, which cultivates trust and enhances understanding of their care plans.


Nurse Practitioners work within our outpatient facilities as part of a collaborative care team that includes a physician, nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and support from registered nurses and care managers.

We collaborate with a network of top-tier specialists and hospitals for specialty and acute care.

Consequently, our healthcare providers can concentrate on clinic-based care:
primary care, care coordination, and population health.

Key Responsibilities:

  • Deliver exceptional primary care services.
  • Conduct office consultations for routine and urgent health issues.
  • Administer annual wellness evaluations and health risk assessments, requiring a comprehensive understanding of health and precise documentation.
  • Coordinate care with other healthcare providers, specialists, testing facilities, and agencies.
  • Lead population health initiatives in collaboration with the Care Team (e.g., ensuring all eligible females receive their evidence-based breast cancer screenings every two years).
  • Assist the care team with phone triage and outreach efforts.
  • Educate patients regarding their health conditions, care plans, and treatment options.
  • Participate in promotional activities for Oak Street Health.
  • Conduct home visits as necessary.
  • Undertake additional duties as assigned.
This position reports to the Center Medical Director and collaborates closely with operational leadership.

Required Qualifications:

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
  • Active, non-probationary state Nurse Practitioner license.
  • Active DEA license.
  • Authorization to work in the U.S.

Preferred Qualifications:

  • Experience in primary care settings: internal medicine, geriatrics, or family medicine.
  • Familiarity with HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) coding, and Health Risk Assessments (HRAs).
  • A passion for teamwork and collaboration across functions.
  • A desire to contribute to an innovative model focused on empirically-guided population health.
  • Bilingual proficiency in relevant areas.
