As a vital member of Strive's clinical team, the Advanced Practice Nurse Specialist will leverage their advanced training and expertise to provide exceptional nursing care for patients with chronic and end-stage kidney disease. This role involves collaborating with a multidisciplinary team, including social workers, care coordinators, pharmacists, dietitians, and physicians, to ensure the highest quality of patient care. The Advanced Practice Nurse Specialist will serve as a patient advocate, navigator, and trusted healthcare provider, ensuring a seamless and supportive experience for patients.
Key Responsibilities
Responsible for achieving patient outcomes and key performance indicators for a designated group of patients. Conduct thorough patient assessments, including history-taking, physical examinations, and diagnostic test interpretation, to develop comprehensive care plans. Participate in clinical rounds and interdisciplinary meetings, presenting patient cases and providing clinical insights. Ensure that care plans adhere to best practices for chronic kidney management, including shared decision-making and advanced care planning. Provide care across various settings, including patient homes and telehealth consultations. Act as a liaison with external healthcare providers to facilitate coordinated care transitions. Maintain required certifications and adhere to professional development standards. Deliver primary and urgent care services to adult and geriatric patients with chronic kidney disease.Qualifications
Master's degree in Nursing or a related field. Minimum of 2 years of experience as a Nurse Practitioner in relevant specialties. Valid state licensure and national board certification. Current DEA license or eligibility to obtain one. BLS or CPR certification. Reliable transportation for patient visits. Proficient in using electronic health records and basic computer skills.About You
