Role Summary:

The part-time Care Management Specialist serves as a temporary member of an interdisciplinary team, assisting elderly individuals, adults with physical disabilities, and those with intellectual or developmental disabilities who are part of Lakeland Care. This role involves collaborating with a Registered Nurse Care Manager to provide care management services to members. The objective is to assist members in navigating healthcare systems and utilizing available resources to enhance their health and wellness through high-quality, person-centered care.

The part-time Care Management Specialist is responsible for coordinating and designing service provisions based on a thorough assessment of the member's needs and desired outcomes.

This position entails working 20 hours per week, typically managing a partial caseload, although this may fluctuate based on organizational requirements. Responsibilities include providing coverage for Care Managers during extended absences, bridging the gap between the departure of an employee and the onboarding of a new hire, or any other tasks as assigned by the Care Manager Supervisor or Program Manager. The part-time Specialist will also facilitate shadowing opportunities and smooth transitions as caseloads change.

Flexibility to cover multiple service areas and travel as necessary is a requirement for this role.

Key Responsibilities & Skills:

  • Conduct comprehensive assessments of members' needs, outcomes, and risks, with follow-up reassessments as necessary.
  • Coordinate care and benefits to ensure a holistic support approach and ongoing access to federal and state programs.
  • Monitor and evaluate outcome-based care plans, considering cost-effectiveness in service authorization and provider selection.
  • Engage in home visits and care conferences with members, their support systems, and providers to assess and reassess long-term care needs.
  • Implement strategies to mitigate risks, promoting members' health, safety, and independence while honoring their rights.
  • Maintain member records in compliance with Department of Health Services contracts and organizational policies.
  • Participate in ongoing training to stay current with federal and state regulations, as well as organizational policies and accepted professional standards.
  • Exhibit strong time management skills to effectively handle workload and caseload logistics.
  • Build relationships within Lakeland Care with the assigned Care Manager Supervisor, team members, and support divisions during coverage periods.
  • Uphold the confidentiality of member information and protected health information as mandated by state and federal regulations, including HIPAA.

Qualifications:

  • Certified Social Worker in Wisconsin with at least one year of experience with Family Care target populations, or
  • A four-year degree or higher in Human Services or a related field with one year of relevant experience, or
  • A four-year degree in another field with a minimum of three years of experience with Family Care target populations.
  • Ability to adapt, prioritize, and thrive in a fast-paced environment.
  • Proficient in computer usage, including typing and data entry.
  • Capability to access members' homes that may not comply with ADA regulations.
  • Ability to lift up to 25 pounds.
  • Valid driver's license, acceptable driving record, and proof of adequate insurance.
