Job Summary:


In this role, you will deliver comprehensive case management services to adult clients receiving welfare across multiple counties. You will manage a designated caseload and create tailored service plans that comply with WFNJ and SAIF regulations.


Key Responsibilities:
Conduct ongoing assessments through regular interactions with clients to evaluate their functioning and progress towards their objectives.

These interactions may include: 1) Home visits; 2) Client telephone consultations; 3) Client office meetings; 4) Visits to client workplaces.

Document client progress and provide crisis intervention and guidance to clients, their families, employers, and other stakeholders as required by the program. Collaborate with supervisors regarding each situation prior to taking action and share client information with the Program Manager for the Monthly Level of Service Report.

Maintain comprehensive case files for each assigned client and connect clients to suitable supportive services when they qualify for Social Security disability benefits and/or medical deferment.
Advocate for clients and assist them with service applications when necessary.
Identify potential employers and create job opportunities tailored to the specific needs of clients, including facilitating their placement in competitive job settings and nurturing relationships with both potential and current employers.
Participate in monthly case management meetings organized by BCBSS and/or other Regional County WFNJ collaborative gatherings.
Assist the Program Manager with monthly presentations at relevant centers as directed.
Communicate client progress, needs, and concerns through case reviews and monthly reports.
Provide transportation for clients as needed.

Qualifications:


A Bachelor's Degree in Social Work or a related field from an accredited institution, along with experience in crisis intervention, counseling, mental health, substance abuse, domestic violence, family therapy, group work, and job development.

A valid driver's license is required, and candidates must be insurable under the agency's automobile insurance policy.
