Your Role

Join us as a Clinical Care Specialist at Honest, where you will be instrumental in delivering exceptional clinical services. You will collaborate with a team of professionals, including registered nurses, pharmacists, social workers, and care coordinators, all part of the Honest community.

This position is remote, but proximity to our operational area is essential for in-home patient support and participation in meetings with Honest team members and partners.

Main Duties:

  • Oversee comprehensive management of patient conditions, from evaluation to treatment, on an ongoing basis.
  • Facilitate care transitions with post-acute facilities.
  • Accurately document patient interactions in an electronic medical record system.
  • Ensure high standards of quality and patient satisfaction in every engagement.
  • Collaborate with the care team to address social determinants affecting patient health.
  • Perform additional responsibilities as required.

Qualifications

  • Completion of a Nurse Practitioner or Physician Assistant program.
  • Active state license and board certification or eligibility for certification within 2 years.
  • BLS certification is mandatory.
  • Preferred experience in phlebotomy and medication administration.
  • Strong understanding of clinical care standards and a focus on enhancing patients' quality of life.
  • Ability to travel to various work locations and maintain a valid driver's license.
