Job Description

We are seeking a highly skilled Registered Nurse to fill this Patient Care Coordinator position. In this role, you will be responsible for ensuring that patients receive high-quality care and support throughout their hospital stay.

Key Responsibilities:

  • Assess patient needs and develop individualized care plans
  • Collaborate with healthcare providers to coordinate patient care activities
  • Ensure timely and effective communication with patients, families, and healthcare teams

Requirements:

  • Degree in Nursing from an accredited institution
  • Licensed RN with Basic knowledge of nursing theory and practice
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare teams

Salary Estimate: $80,000 - $105,000 per year
