Job Description
We are seeking a highly skilled Registered Nurse to join our team as a Clinical Care Coordinator. As a key member of our interdisciplinary team, you will be responsible for providing exceptional patient care and collaborating with other healthcare professionals to achieve optimal outcomes. Key responsibilities include:
- Performing thorough and ongoing holistic nursing assessments to identify current and potential problems
- Developing individualized plans of care that address patient needs and concerns
- Collaborating with patients, family members, and healthcare teams to meet patient needs and outcomes
- Implementing and revising plans of care as needed
Required Skills and Qualifications
To be successful in this role, you will need:
- A minimum of 2 years of critical care or emergency room experience
- A graduate degree from an accredited school of nursing
- Current licensure in the State of New York
- Basic knowledge of nursing theory and practice
- Strong communication and interpersonal skills
