Community Health Outreach Coordinator

Cornerstone Family Healthcare is a leading healthcare provider in the region, offering a range of services to promote community well-being. We are currently seeking a Community Health Outreach Coordinator to join our team in Kingston.
Estimated Salary: $60,000 - $80,000 per annum (dependent on experience)
About the RoleWe are looking for an experienced professional to coordinate outreach activities in Ulster and Dutchess Counties. As a key member of our mobile van based outreach program, you will work closely with the Syringe Exchange Program (SEP) Coordinator to deliver syringe exchange, testing, counseling, and referrals to individuals at risk.
Main Responsibilities:
- Conduct outreach to individuals at risk in community settings, including shelters, soup kitchens, and other organizations;
- Develop and maintain partnerships with community providers to raise awareness of health issues and HIV/HCV risks;
- Provide HIV/HCV counseling, testing, referrals, IDIs, and partner services to clients at risk and their partners;
- Offer ESAP vouchers, PrEP information, and opioid overdose prevention resources to increase awareness and risk reduction skills;
- Ensure compliance with chart requirements, testing protocols, data entry, and agency policies and procedures;
- Maintain proper vehicle maintenance and track mileage;
- Attend training sessions, including Opioid Overdose Prevention Program (NARCAN), as assigned;
- Perform HIV/HCV testing and counseling to individuals at risk;
- Provide referrals to drug treatment, benefits, medical treatment, and HIV/HCV services, where applicable;
- Follow up on all referrals;
- Ensure client charts are complete and accurate, adhering to program standards and goals;
- Enter client data into the agency's AIDS Institute Report System (AIRS) in a timely manner;
- Assess community needs and ensure services are effective and reaching the priority population;
- Participate in supervision with the SEP Coordinator on a weekly basis;
- Assist with program and van coverage as needed;
- Attend department, All Staff, and required meetings;
- Maintain confidentiality of patient information, financials, and employee relations;
- Perform other related duties as assigned.
We are seeking an experienced professional with a strong background in outreach, HIV/AIDS counseling, and disease prevention. The ideal candidate will have:
- Bachelor's Degree in education, health, human services, or a related field, or equivalent experience;
- Minimum one year experience working with individuals at risk, drug treatment, knowledge of HIV/AIDS, harm reduction, mental health, and/or community resources.
