Care Coordinator

At Lamoille Health Partners, we are seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will work closely with patients and their families to identify their health goals and create a care plan that supports them in achieving those goals.
Key Responsibilities:
- Coordinate patient care with members of the integrated healthcare team.
- Assess the social, emotional, familial, and financial factors impacting the patient's ability to cope with problems of daily living.
- Develop a self-management plan for the patient's condition and assist them in achieving their goals.
- Provide patient and family educational materials and self-management tools on issues of health maintenance and management of chronic conditions.
- Collaborate with cross-departmental teams and/or members of partner agencies to develop care plans.
Requirements:
- Bachelor's degree in Social Work or related field.
- LICSW or LMHC preferred.
- At least two years of experience in a primary care office or equivalent.
