Care Coordinator

We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health (ISBH). As a Care Coordinator, you will play a critical role in providing exceptional care to children, youth, and families with significant behavioral health needs.
Key Responsibilities:
- Collaborate with families to identify care coordination needs and develop personalized care plans.
- Engage with community resources to provide support and services to families.
- Build and maintain positive relationships with children, youth, and families to ensure successful integration of care plans.
- Participate in program evaluation and attend team and program planning meetings.
- Stay current with training requirements, including High Fidelity Wraparound and Cultural Humility.
Requirements:
- Experience providing services to children and families connected to behavioral health, child welfare, or developmental disabilities.
- Three years of experience with a high school diploma or equivalent, or two years with an associate degree or bachelor's degree, or one year with a master's degree or higher.
- Knowledge and experience in Hi-Fidelity Wraparound preferred.
- Two years of experience in a coordinated supportive services or care coordination role preferred.
