Overview:

As part of New York City's initiative to combat homelessness, the Department of Homeless Services has established Homebase Homeless Prevention Community Resource Centers across the city. These programs are designed to assist families and individuals facing homelessness or at imminent risk of it, helping them to create a sustainable plan for long-term housing stability.

In your role as the Case Management Team Leader, you will oversee the Case Management and Housing Specialist staff, ensuring the provision of comprehensive, community-oriented services.


Key Responsibilities:
  • Provide emergency crisis intervention for individuals and families on the brink of housing instability, ensuring timely support to avert shelter entry or re-entry.
  • Supervise and mentor Case Managers and Housing Specialists, ensuring the delivery of high-quality services tailored to client needs.
  • Collaborate with senior management to assess program effectiveness, identify service delivery gaps, and develop actionable solutions.
  • Maintain accurate documentation of services provided through CARES database reports, case conferences, and team meetings.
  • Engage in outreach initiatives and present to community organizations to enhance program visibility and increase enrollments.
  • Conduct regular supervision sessions with staff, focusing on professional development and skill enhancement.

Qualifications:
  • Bachelor's degree in social work or a related field, or equivalent experience.
  • Exceptional verbal, written, and negotiation skills, with a background in advocacy and mediation.
  • Proficiency in Microsoft Office applications.
  • Availability for evening and weekend coverage as required.
