Position Overview

In the role of Senior Clinical Case Manager, you will report directly to the Director of Supportive Housing and oversee the case management team, including interns. Your responsibilities will encompass providing case management services to residents of Woodycrest Supportive Housing, who may be dealing with substance use and/or mental health issues. The services you provide will include individual counseling, benefits coordination, connections to community resources, intensive care referrals, employability assessments, and enhancing housing stability.

Key Responsibilities:
  • Manage a caseload of tenants, ensuring it does not exceed 15 households at any given time.
  • Provide bi-weekly supervision and support to case managers and student interns.
  • Conduct regular audits of case files and documentation.
  • Perform assessments, develop individualized service plans, and offer referrals and ongoing support to assist tenants in achieving their personal goals related to housing stability and overall well-being.
  • Act as a primary resource for supportive services available to tenants, connecting them with community-based services that align with their Individualized Service Plans (ISPs).
  • Facilitate various support groups and workshops aimed at promoting independent living.
  • Serve as a liaison with medical, mental health, and substance abuse service providers to ensure tenants receive the care they deserve.
  • Assist with educational and employment linkages.
  • Support tenants in overcoming barriers that may hinder timely rent payments.
  • Collaborate with property management to assist tenants in maintaining permanent housing.
  • Advocate for residents in various aspects of their lives, offering assistance as needed.
  • Participate in weekly supervision meetings with the Director of Supportive Housing, departmental staff meetings, interdepartmental team meetings, and case conferences.
  • Utilize the AWARDS electronic case management system for documenting case information and maintaining compliance with organizational policies and regulations.
Qualifications:
  • Master's degree in Social Work or a related field is required.
  • A minimum of 3 years of experience in case management, supervision, service coordination, assessment, counseling, and crisis intervention.
  • Familiarity with NYC benefits programs, rental assistance resources, and housing subsidy programs is preferred.
  • Excellent oral, written, and negotiation skills.
  • Strong organizational abilities and attention to detail.
  • Capacity to work independently as well as collaboratively within a team.
  • Proactive, dependable, and resourceful with a keen eye for detail.
  • Knowledge of family systems approach and experience with Motivational Interviewing and other best practices is preferred.
  • Proficiency in Microsoft Office applications.
  • A valid NYS driver's license is a plus.
  • Availability for evening and weekend coverage as necessary.
Compensation: $31.25/hour
