Role Summary:

In alignment with New York City's initiative to combat homelessness, the Department of Homeless Services has established Homebase Homeless Prevention Community Resource Centers across the city.

The Homebase initiative aims to support families and individuals who are either experiencing homelessness or are at high risk of becoming homeless, guiding them towards sustainable housing solutions.

Homebase provides assistance in navigating the complex eviction landscape, offers financial and budgeting counseling, aids in securing public benefits, provides short-term financial support, and facilitates relocation when necessary.

As the Supervisor of Case Management, you will oversee the Case Management and Housing Specialist teams, ensuring the provision of comprehensive, community-focused services.

Your leadership will be crucial in delivering emergency crisis interventions for individuals and families facing immediate housing challenges, ensuring they receive timely support to avert shelter entry or re-entry.

Your goal will be to stabilize clients' housing situations and enhance their self-sufficiency.

Your duties will encompass addressing program-related issues, conducting regular supervisory sessions, delivering community presentations, monitoring program activities, achieving mandated enrollment targets, and ensuring the efficient execution of case management operations.

When necessary, you will also manage a designated caseload.


Key Responsibilities:

  • Supervise Case Managers and Housing Specialists to guarantee the delivery of high-quality services to clients.
  • Provide direct intervention for complex cases and assist staff in connecting clients with essential entitlements to maintain their housing.
  • Collaborate with the Executive Director and Assistant Executive Director to assess program effectiveness, identify service delivery gaps, and contribute to developing solutions for performance challenges.
  • Ensure accurate documentation of services through CARES database reports, case conferences, team meetings, supervision, and case record audits, while collecting, analyzing, and reporting team statistics.
  • Engage in outreach efforts and conduct presentations to community organizations to boost program enrollments.
  • Deliver and document regular, scheduled supervision for assigned staff, identify development opportunities, and co-facilitate training sessions with team members.
Qualifications:

  • Bachelor's degree in social work or a related field, or equivalent experience.
  • At least 2 years of experience in case management, assessment, counseling, and crisis intervention preferred.
  • Supervisory experience in a case management environment is highly desirable.
  • Familiarity with family systems approaches to practice is preferred.
  • Strong verbal, written, and negotiation skills, with experience in advocacy and mediation.
  • Excellent organizational and documentation skills.
  • Ability to thrive in a fast-paced environment.
  • Initiative, dependability, resourcefulness, and attention to detail are essential.
  • Strong organizational skills to manage multiple priorities effectively and within time constraints.
  • Valid US driver's license is a plus.
  • Proficiency in Microsoft Office applications.
  • Availability for evening and weekend coverage as required.
