Job Overview

Palm Medical Centers is a rapidly expanding organization seeking a skilled Clinical Coordinator with a strong background in leadership to enhance our operations.

The Clinical Coordinator is responsible for a diverse range of activities and oversees the care plans for patients in hospitals and nursing facilities, ensuring a seamless transition from admission to discharge while maintaining high standards of care.

This role reports directly to the Senior Director of Case Management within the Utilization Management Department.

Key Responsibilities:

  • Facilitate the admission of new clients by reviewing documentation and conducting orientation sessions.
  • Track daily hospital and nursing home admissions and discharges for both statistical analysis and cost management.
  • Assess client needs through intake interviews; determine the necessity for therapeutic medical, psycho-social, and psychiatric evaluations, and review treatment objectives and plans.
  • Develop treatment programs by organizing schedules and coordinating services; arrange necessary resources, including transportation.
  • Monitor patient cases by confirming attendance; evaluate treatment effectiveness; advocate for essential services and resources; provide support during crises.
  • Maintain comprehensive client records by reviewing case notes and documenting progress.
  • Communicate with management regarding patients discharged from inpatient facilities to ensure appropriate follow-up care.
  • Obtain regular updates on nursing home patients.
  • Identify patients for potential subrogation and cost allocation considerations.
  • Coordinate patient transfers as directed by the Medical Director or Primary Care Provider.
  • Facilitate direct admissions to hospitals and placements in nursing homes.
  • Assist Primary Care Providers in patient placements at Skilled Nursing Facilities.
  • Monitor length of stay for both hospital and nursing home patients and coordinate discharge arrangements.
  • Report on client progress through weekly interdisciplinary meetings, sharing results and challenges with the therapeutic team and family members.
  • Enhance staff capabilities by providing educational resources and balancing work with learning opportunities.
  • Improve treatment outcomes by analyzing processes, implementing changes, and updating policies and procedures.
  • Enhance financial performance by monitoring results, identifying trends, and advising management on necessary actions.

Qualifications:

  • Demonstrated experience in case management, including roles as an RN, medical or mental health care manager, or similar positions.
  • Strong understanding of case management principles, management practices, and reimbursement processes.
  • Prior experience with the psychological aspects of patient care.
  • Excellent communication and organizational skills.
  • Ability to manage time effectively and multitask.
  • Familiarity with emerging professional and technical knowledge.
  • Strong problem-solving skills and a compassionate approach to teamwork.
  • Knowledge of CPT codes and ICD-10 terminology.
  • Bilingual candidates are preferred.
  • Familiarity with InterQual or Milliman Care Guidelines is a plus.

Minimum Requirements: Active Florida RN license and a minimum of 3 years of experience as a Clinical Coordinator.
