**Job Summary**

We are seeking a highly organized and detail-oriented Schedule Coordinator to join our team. As a Schedule Coordinator, you will play a critical role in managing patient referrals and visit schedules, ensuring that our clinicians have the necessary resources to provide exceptional care.

**Responsibilities**

  • Utilize electronic medical records (EMR) and scheduling tools to efficiently schedule visits with field staff, taking into account business needs, patient needs, and employee needs.
  • Process workflow for requested, missed, rescheduled, reassigned, declined, and delivered visits.
  • Monitor pending referrals daily and assign licensed professionals and patient care managers for all scheduled visits.
  • Communicate daily with field staff regarding any visits in late, pending, or incomplete status for resolution as appropriate.
  • Run scheduling request reports daily, complete requests as necessary for Medicare week, and monitor productivity and distribution of regular visits for caregiver optimization.
  • Review unverified visit reports daily and communicate directly with field staff, patient care managers, and clinical directors as needed.
  • Monitor action screens and process all assigned tasks, coordination notes, and administrative tasks in a timely manner.
  • Assist in coordinating scheduling and tracking all supervisory visits and therapy reassessment visits, as required by regulatory requirements.
  • Attend and participate in annual in-service programs, workshops, continuing educational programs, seminars, and other training events.
  • Communicate professionally with all employees and external contacts, including physicians, referral sources, and other stakeholders.

**Requirements**

  • High school education or equivalent.
  • Minimum one year of scheduling experience in a healthcare setting using an online scheduling system.
  • Exceptional organizational, customer service, communication, and decision-making skills.
  • Working knowledge of state and federal regulations governing OASIS visits, supervisory, and reassessment visits.
