About the Role

We are seeking a highly skilled and compassionate Client Care Coordinator to join our team at Comprehensive Home Health Solutions. As a Client Care Coordinator, you will play a vital role in ensuring the highest level of care for our clients and their families.

Key Responsibilities
  • Client Intake and Assessment: Conduct thorough intake assessments with potential clients and their families, assess their care needs, and develop personalized care plans.
  • Caregiver Assignment and Scheduling: Match clients with qualified care providers, coordinate and schedule caregivers for home visits, and monitor and supervise care providers to ensure adherence to care plans.
  • Client Relations: Act as the primary point of contact for clients and their families, addressing any concerns or questions, and providing timely follow-up on client inquiries and concerns.
  • Caregiver Support and Management: Provide ongoing support to caregivers, offer guidance and resolve scheduling or performance issues, and work with the HR department to ensure caregivers meet all compliance and certification requirements.
  • Documentation and Compliance: Maintain detailed client records, ensure all documentation complies with company policies, local and federal regulations, and privacy laws, and monitor and report any changes in clients' health status or care needs.
  • Coordination of Services: Collaborate with other care providers to ensure a holistic approach to care, arrange for additional services as needed, and ensure that clients receive the highest level of care.
  • Quality Assurance and Improvement: Develop and implement quality control processes to ensure excellent client care, conduct regular quality assurance assessments, and address any issues or deficiencies identified through quality assurance assessments.
Requirements
  • Education: High school diploma or equivalent required; Bachelor's degree in healthcare, social work, or a related field preferred.
  • Experience: Previous experience in home care, healthcare, or customer service, particularly in a coordinator or supervisory role.
  • Skills: Strong communication and interpersonal skills, excellent organizational and multitasking abilities, problem-solving skills, and proficiency with scheduling software and basic office technology.
  • Familiarity: Familiarity with local and federal regulations regarding home care services.
  • Availability: Ability to work flexible hours, including evenings or weekends, as needed.
