Responsibilities:
  • Client Intake and Assessment: Conduct thorough intake assessments with potential clients and their families to understand their care needs and develop personalized care plans.
  • Caregiver Assignment and Scheduling: Match clients with qualified care providers, ensuring compatibility and continuity of care, and coordinate and schedule caregivers for home visits.
  • Client Relations: Act as the primary point of contact for clients and their families, addressing any concerns or questions, and provide timely follow-up on client inquiries and concerns.
  • Caregiver Support and Management: Provide ongoing support to caregivers, offering guidance and resolving scheduling or performance issues, and work with the HR department to ensure caregivers meet all compliance and certification requirements.
  • Documentation and Compliance: Maintain detailed client records, including care plans, progress notes, and communication logs, and ensure all documentation complies with company policies, local and federal regulations, and privacy laws.
  • Coordination of Services: Collaborate with other care providers to ensure a holistic approach to care and arrange for additional services as needed.
  • Quality Assurance and Improvement: Develop and implement quality control processes to ensure excellent client care and conduct regular quality assurance assessments to evaluate the effectiveness and appropriateness of care services.
Qualifications:
  • Education: High school diploma or equivalent required; Bachelor's degree in healthcare, social work, or a related field preferred.
  • Experience: Previous experience in home care, healthcare, or customer service, particularly in a coordinator or supervisory role.
  • Skills: Strong communication and interpersonal skills, excellent organizational and multitasking abilities, problem-solving skills, and proficiency with scheduling software and basic office technology.
