Key Responsibilities:

  • Fosters collaborative relationships with healthcare professionals including physicians, nurses, social workers, and community partners through effective communication and educational initiatives to ensure seamless patient transitions.
  • Participates in team meetings, discharge planning sessions, and outcome evaluation discussions, serving as a resource for home care services. Engages with healthcare providers on complex cases to confirm that care requirements are adequately addressed in a home environment.
  • Upon receiving referrals, reaches out to patients to inform them about agency service offerings, ensuring that the care plan aligns with hospice eligibility and adheres to regulatory standards.
  • Gathers and documents medical, social, and reimbursement details from hospitals and post-acute settings to initiate services provided by Advocate Health at Home, including hospice, infusion, enteral, pediatric, DME, and respiratory therapy.
  • Informs patients and healthcare providers about financial coverage and responsibilities related to the services ordered, providing necessary information prior to discharge from facilities when possible.
  • May follow up with Advocate Health at Home hospice patients who are readmitted to hospitals or post-acute settings to assess the necessity for ongoing services.
  • Exhibits the knowledge and skills essential for delivering age-appropriate care, demonstrating an understanding of growth and development principles across the lifespan. Capable of assessing and interpreting patient data to identify specific care needs.
  • May assist in training and orienting new hires.
  • May oversee the admission process for hospice services, which includes obtaining medical history, implementing care plans, and managing symptoms associated with terminal diagnoses to enhance comfort during end-of-life care.
  • Collaborates with the business development team and product line director to drive admission growth and expand market presence.

Minimum Education and Experience Required:

License/Registration/Certification: Registered Nurse with a valid driver's license.

Issued by: State of practice.

Level of Education: Graduate from an accredited nursing program; BSN preferred.

Field of Study: Nursing.

Years of Experience: Typically requires 2 years of clinical nursing experience, particularly in discharge planning or care management.

Type of Experience: Preferred experience in hospice care.

Minimum Knowledge, Skills, and Abilities:

  • Strong verbal and written communication skills, with the ability to interact effectively with both internal and external stakeholders.
  • Excellent organizational and analytical thinking abilities.
  • Proficient understanding of hospice regulations and reimbursement frameworks, including Medicare, Medicaid, and commercial payers.
  • Strong technical skills.
  • Able to critically assess patient conditions to aid in developing post-acute care plans.
  • May involve training patients and families on equipment usage (e.g., enteral and infusion pumps).
  • Familiarity with electronic health record systems, preferably Epic.
  • May assist in processing referrals for hospice programs.
  • Availability for on-call duties, including weekends and holidays.
  • Skilled in fostering positive patient experiences, service recovery, and conflict resolution to enhance collaboration among healthcare teams, patients, and caregivers.

Physical Requirements and Working Conditions:

  • Responsible for patient admissions and care coordination as necessary. Must be able to travel and have flexible availability to meet patient needs, including working a minimum of one weekend per month.
  • If the position involves direct patient care, the ability to lift up to 35 pounds independently is required. For heavier lifts, appropriate patient handling equipment should be utilized with assistance when available.
