We are seeking a highly skilled and compassionate Social Services Coordinator to join our team at Sharon S. Richardson Hospice. As a key member of our interdisciplinary team, you will provide essential social and emotional support to our patients and their families, addressing the complex needs that impact their ability to achieve their goals.
Key Responsibilities- Assessment and Counseling: Conduct thorough social service assessments, provide counseling, education, and ongoing evaluation of clients receiving services under a medically approved Plan of Care.
- Documentation and Communication: Document all findings, plans, interventions, and client progress in the clinical record, and communicate effectively with the healthcare team to ensure seamless care coordination.
- Crisis Intervention and Support: Assist in crisis intervention, provide emotional support, and help patients and families navigate the healthcare system.
- Community Resource Referral: Identify and connect patients and families with community resources, services, and support groups to enhance their quality of life.
- Team Collaboration and Quality Improvement: Participate in care conferences, team meetings, and quality improvement activities to ensure excellent patient care and continuous improvement of our services.
- Education and Experience: Master's degree in social work and a minimum of one year of experience in a healthcare setting, preferably in hospice or palliative care.
- Skills and Qualities: Excellent written and verbal communication skills, strong interpersonal skills, ability to work independently, and a commitment to providing compassionate and patient-centered care.
- Certifications and Licenses: Current CPR certification, as applicable, and a valid driver's license with reliable transportation.
This is a fast-paced, dynamic environment that requires flexibility, adaptability, and a strong sense of teamwork. You will work closely with our interdisciplinary team to provide comprehensive care to our patients and their families. As a Social Services Coordinator, you will have the opportunity to make a meaningful difference in the lives of others and contribute to the mission of Sharon S. Richardson Hospice.
